How to Begin Services

Finding the Right Home and Support Team

Choosing a provider is an important decision. Our admission process helps determine whether AltruisticLoving Care can safely and appropriately meet the individual’s needs.

Step 1: Confirm APD Eligibility

The individual should be eligible for services through Florida’s Agency for Persons with Disabilities or beactively working with APD to determine eligibility.

Families who are beginning the process should contact the appropriate APD regional office for currenteligibility and application requirements.

Step 2: Contact the Waiver Support Coordinator

The individual’s waiver support coordinator can help:

  • Identify authorized services
  • Confirm available funding
  • Discuss residential placement
  • Submit referrals
  • Coordinate provider interviews
  • Review service needs
  • Assist with support-plan updates
  • Coordinate the transition process

Step 3: Submit a Referral

Referrals may be submitted by:

  • The individual
  • A parent or family member
  • A legal guardian
  • A waiver support coordinator
  • An authorized case manager
  • A hospital or facility discharge planner
  • Another approved representative

 

The initial referral should include enough information to help us understand the individual’s support needswithout sending unnecessary protected health information through an unsecured website form.

Step 4: Initial Review

Our team reviews information related to:

  • Requested services
  • Funding authorization
  • Daily living needs
  • Mobility
  • Communication
  • Medical needs
  • Medication support
  • Behavioral needs
  • Supervision level
  • Staffing requirements
  • Personal preferences
  • Compatibility with current residents
  • Environmental and accessibility needs

 

Additional records may be requested through an approved and secure method.

Step 5: Meet and Tour

The individual and authorized support-team members may be invited to:

  • Tour the home
  • Meet members of the staff
  • Discuss routines and expectations
  • Review bedroom availability
  • Ask questions
  • Discuss preferences
  • Learn about meals and activities
  • Review transportation needs
  • Discuss transition concerns

 

Tours are scheduled by appointment to protect the privacy and routines of current residents.

Step 6: Compatibility and Safety Review

Before admission, we determine whether the placement can appropriately meet the person’s needs.

Considerations may include:

  • Required staffing ratio
  • Accessibility
  • Medical complexity
  • Behavioral supports
  • Evacuation ability
  • Gender and bedroom arrangements
  • Compatibility with current residents
  • Transportation needs
  • Available service authorization
  • Environmental needs
  • Staff training requirements

 

A referral does not guarantee admission.

Step 7: Transition Planning

When a placement is approved, we coordinate a transition plan with the individual and authorized support-team members.

The plan may address:

  • Proposed move-in date
  • Medication transfer
  • Personal belongings
  • Medical appointments
  • Transportation
  • Daily schedule
  • Preferred foods
  • Communication methods
  • Behavioral supports
  • Family contact
  • Community activities
  • Employment or day services
  • Emergency information
  • Required documents
